We are aware of the current issue: Probationary applications not generating correctly for Initiation and Reinstatement activities. It is generating a Membership Form 15 instead of the correct Probationary Form 13. We are actively working to resolve the issue. In the meantime, please use this guide to print the correct Probationary Form 13 to obtain signatures.
